We (parents) accept authority of Long Bay College and all the provisions as set out in the Tuition Agreement and are aware that Long Bay College will act according to the Code of Practice.  By ticking the box we give permission for our son/daughter to attend any class/sports/cultural trip which takes place during the normal school day.
I give my consent for the school to obtain from other schools and agencies, information relevant to this students„1¤7 education at Long Bay College.
I give my consent for Long Bay College to give information to The New Zealand Qualifications Authority and appointed Agents acting on behalf of Long Bay College.
Renewals of visas must be done before they expire and evidence provided to Long Bay College.  Failure to do this may mean that the student will not be able to attend Long Bay College.
